The Pharmacy at the NRH provides a comprehensive pharmacy service to all patients, carers and staff within all Rehabilitation Programmes at the NRH.

The Pharmacy Department has a key role in Interdisciplinary Team Working at the NRH:

  • Pharmacy staff reconcile all medications on patients’ admission and discharge to ensure that the medication list is accurate at transitions of care
  • Reviews of all prescriptions are undertaken by Pharmacy to optimise medication therapy
  • Pharmacists attend Consultant ward rounds to advise proactively on medications at point of prescribing
  • Pharmacy staff participate in a weekly anti-microbial stewardship round with the Consultant Microbiologist
  • Pharmacy dispenses medication for patients for therapeutic weekend leave
  • Pharmacists counsel patients on their medications prior to discharge on request and provide an individual medication list to each patient

  • Provides both individual and group medicines education sessions for patients and carers.
