Rehabilitation Nurse: Nursing staff work together to assist patients to achieve functional independence, while continuing to maintain a high quality standard of care. The interdisciplinary team (IDT) approach is an integral part of successfully delivering results and accompanying our patients on their rehabilitation journey. It supports staff to enhance the patient experience by involving the patient in setting their goals and respecting what they hope to achieve from their rehabilitation programme. This work is undertaken in collaboration with our IDT colleagues.

An example of an IDT initiative introduced within the Brain Injury Programme is the IDT ‘breakfast club’ which provides a new opportunity for patients and staff to work in a cohesive, collaborative manner. It enables a series of person-centred goals to be achieved in a functional and meaningful way. IDT members include Nursing, Speech and Language Therapy, Occupational Therapy, Physiotherapy and Catering. Members of Psychology attend as needed.

Nursing staff provide quality care, support and encouragement throughout your stay and provide an important communication link with other members of the interdisciplinary rehabilitation team.

Nursing staff help you to practice what you are learning in therapies and teach you and your family how to manage your personal care.

Your interdisciplinary team (IDT) includes staff from Nursing, Medical and Therapy services, Healthcare Assistants and other support services staff, such as Catering and Administration Staff.

The IDT Team will work together with you to deliver successful results on your rehabilitation journey.  This enhances patient experience by involving you in setting your goals and respecting what you hope to achieve from your rehabilitation programme.
